Schedule of Benefits

Enrolled Members are entitled to:

Eye Examination: Once every 12 months.

Lenses: Covered in full once every 24 months. One pair of standard single vision, lined bifocal, lined trifocal.

Frames: Once every 24 months. Your choice from hundreds of covered in full, fashionable frames displayed in each provider office.

Contact Lenses: Once every 24 months (instead of lenses and frame).

Tint: Tint for plastic lenses, solid or gradient, available at no cost on covered pair of prescription lenses.

Computer (VDT) Glasses: Members are entitled to an eye exam and computer glasses-standard frame and single vision, standard bifocal or standard trifocal lenses, specially coated for computer use; once very 12 months.

Cosmetic Options: Available at special member prices.

Copayments: No copayment is required with this plan (provided all selections are within covered benefits).

How to Use Your Plan

To make an appointment, call VSP at 1-800-877-7195. A Member Services Representative will schedule an appointment for you to see a provider at the location most convenient to you.
